Chronic Disease Management Program
Once you have been diagnosed with a chronic (in some cases this may be various forms of cancer), or terminal illness you may be eligible to Medicare’s ‘Chronic Disease Management’ program. It involves meeting with your GP to see if you are eligible, and if so, developing a GP Management Plan. This generally involves a Medicare rebate up to a maximum of five services per patient each calendar year eg visits to a physiotherapist, occupational therapist, exercise physiologist, psychologist, dietician etc. There may still be some costs incurred by you, depending on the service provider.

Prosthesis

Reimbursement Program

Provides reimbursement of up to $400 for a new or replacement external breast prosthesis for Australian women who have had a single mastectomy as a result of breast cancer, and up to $800 for a bi-lateral mastectomy. A claim can be made every two years.

If you require a new prosthesis within the 2 year period, due to cancer treatment or another medical condition, you may be eligible for a further reimbursement.

Transport for Patients

Illawarra Cancer Carers provide cancer patients with low-cost transport for treatment/medical appointment on a strict ‘needs’ basis, & only if no other transport is available. Transport is arranged via medical practitioners referral through the Wollongong Hospital.
